Dear Student,

Thank you for choosing Tri-State Medical Training Service Inc., as an educational service for Phlebotomy classes.  Our certifing agency is the National Healthcareer Association, and we provide an extensive curriculum into the field.  The classes are six weeks in duration and upon successful completion you will receive certification as a Phlebotomy Technician, in your chosen field.

Requirements for the course are as follows, High School Diploma or higher, one letter of recommendation (e.g. Employer, school, church, friend, etc.), and a copy of current immunization records, (PPD/TB, TETANUS, MMR, HEP B). Also, please forward resume if available.

At the time of registration a brief assessment test will be given and a non-refundable
sixty five-dollar ($65.00) registration fee is required.  Actual fee for the training is seven hundred fifty dollars ($750.00). Payment in full is required to hold your seat in the upcoming training class.  Class size limited to twelve (12) students per training class. 

Class requirements are as follows, 75% is passing, with not more than one class missed, but made up.  All weather related class cancellations will be announced on Fox 96.7 FM & 103.1 FM.  Also twenty-five (25) successful venipunctures and ten (10) successful capillary punctures are required for certification.

There are eighty hours that are required for your phlebotomy technicians’ certificate.  Unsuccessful completion of the course will require a retake at a flat fee of five hundred fifty dollars ($550.00). The exams must be taken immediately following the class unsuccessfully completed.

Tri-State Medical Training Service Inc. does not provide job placement, but available
positions will be made known to high achievers.

Classes and certification exams are held at the Port Jervis location. Classes meet Monday & Wednesday from 5pm to 8pm, Tuesday & Thursday from 9am to 12pm and Saturday from 10am to 4pm, for a six week period.     You must be on time!!!

Checks are no longer accepted, but if taken and returned by the bank there will be a $65.00 fee for the inconvenience. (NO EXCEPTIONS)

Receive a TSM tee shirt, if or when you refer a student to the class and full payment has been made.

There will be a $25.00 (twenty-five dollar) fee for any NO SHOW APPOINTMENTS without a cancellation phone call.

There is a textbook required for the Phlebotomy class, which can be purchased at any local bookstore. PHLEBOTOMY HANDBOOK, eigth edition, by Diana Garza. We have a zero tolerance policy on CHEATING.  If caught or suspected of cheating on any quiz, test, or exam, you will be excused from the class without refund of any money paid.
